The dialysis machine is the start of the bill
4008S classix, Dialog iQ and Surdial X make sense only inside a funded treatment service. Recurring supplies and water can outweigh the capital deal.

The decision at a glance.
- Start here
- Use a locally supported Fresenius 4008S classix as the conventional-HD capability benchmark. Compare complete supply and service offers; the focused platform is not automatically the lowest-cost installation.
- Spend more when
- Consider Dialog iQ for needed biofeedback, HDF or single-needle capability. Surdial X is an HD/HDF alternative where its configured package and Nipro support fit better. Fund training and required supplies alongside added functions.
- Watch the budget
- Include water treatment and distribution, chemical and microbiological monitoring, approved bloodlines, concentrates, filters and disinfectants. Price Dialog iQ's dedicated DiaStream iQ lines explicitly at the same treatment volume as competing offers.
- Check the support
- Obtain service response, parts and consumable replenishment commitments at the hospital. A current regional product page does not establish support for an old donated serial.
Ask the supplier
- What complete per-treatment basket does this modality require?
- Who specifies and maintains the water system and quality monitoring?
- How will treatments continue when one station or a supply shipment is unavailable?
Buy a treatment service, then count the stations
A donated machine can have a zero-dollar invoice while the service still needs a water system, testing, power, consumables, clinical staff, waste handling and repairs. If those recurring commitments depend on a donor who has not funded them, the installation is incomplete. Set the weekly treatment volume before deciding how many machines the grant should buy.
Separate installation from recurring treatment costs and downtime cover. Assign freight, preventive maintenance, water-system servicing, parts and repair travel to a named budget or supplier. This makes the three platforms comparable even when vendors divide capital and consumable charges differently.
What earns the capability premium?
4008S classix gives a conventional HD program a focused starting comparison, with online clearance monitoring and dialysis-fluid filtration documented in current regional material. It is not automatically the cheapest installation. A Dialog iQ or Surdial X supply contract may produce a better overall service even when its console has more capability.
Dialog iQ's bioLogic Fusion uses systolic blood pressure and relative blood volume to calculate ultrafiltration rate. That is a concrete function for the clinical team to evaluate, rather than a generic 'advanced' label. Its HDF and single-needle capabilities, and Surdial X's documented HD/HDF workflows, are reasons to compare actual configurations when the program needs them. Regional approval and the purchased options still define what is available.
If those functions will remain unused, a supported HD-focused system may be better value. If HDF or biofeedback is part of the plan, fund the training, water requirements and consumables needed to use it. The manufacturer evidence establishes capability, not a comparative outcome benefit, lower staffing requirement or failure-rate advantage.
The water installation is part of the purchase
CDC guidance makes chemical and microbiological water quality, treatment, storage and distribution part of the dialysis system. A household filter or a reverse-osmosis label is not evidence that the installed supply is adequate. Have the responsible clinical and technical leads specify source-water variation, treatment capacity, distribution, disinfection and monitoring together.
Apply current standards and local requirements to that complete installation, with a defined response to outages and unsuitable water. A cheaper machine cannot compensate for an unvalidated supply. A more capable machine does not remove that dependency.
Make the recurring bill comparable
Build the same treatment basket for each proposal: approved bloodlines, dialyzer, concentrates, filters, disinfectants and other required supplies, plus scheduled parts and technical labor. Dialog iQ's dedicated DiaStream iQ bloodlines should be visible in that basket. Use equivalent treatment volumes and delivery assumptions rather than importing a low online consumable price from another market.
Small recurring differences accumulate. A difference of one currency unit over 10,000 treatments becomes 10,000 units; that is arithmetic, not a claimed price difference between these machines. It explains why a capital discount should be assessed against a written multi-year consumables and service proposal. The missing price evidence here does not support naming a universal cost winner.
A refurbished unit fits best inside a supported fleet
A hospital already maintaining a platform may gain useful capacity from a documented refurbishment. Assess its service history, enabled configuration, acceptance tests and parts horizon against that existing system. The same unit is a different proposition for a new program without technicians or consumable stock.
Plan how treatment continues when one machine is unavailable. Fewer fully supported stations with dependable water, supplies and repair cover can provide more useful service than a larger row of clearance machines. Expand when the operating commitments can expand with the hardware.
Compare the options.

Fresenius 4008S classix
HD-focused comparison point
- Best for
- A program whose specified service is conventional hemodialysis and whose local Fresenius support is established.
- Why consider it
- Current regional manufacturer material describes an HD platform with online clearance monitoring and dialysis-fluid filtration.
- The trade-off
- A current regional page is not proof of availability in every country or support for an old donated serial. Machine water requirements do not describe the entire water plant.
- Price & package
- Quote required for machine, consumables, water system, installation, training and service at an agreed treatment volume.
- Support
- Current regional product information exists. Written country/serial-specific service and supply commitments are still required.
B. Braun Dialog iQ
Biofeedback and broader therapy capability
- Best for
- A supported dialysis program whose clinical team wants the configured biofeedback functions or HDF capability and can sustain the required training, water system and consumables.
- Why consider it
- Its bioLogic Fusion function uses systolic blood pressure and relative blood volume to calculate ultrafiltration rate. The manufacturer also documents HDF and single-needle therapy, giving a concrete basis for a capability comparison.
- The trade-off
- These functions need a clinical use case, country approval and confirmation of the purchased configuration. Their presence does not establish superior outcomes or reduced staffing. Price the dedicated DiaStream iQ bloodlines explicitly.
- Price & package
- Quote required; request separate capital, installation, recurring per-treatment and service costs.
- Support
- Manufacturer portfolio and catalog are current; recipient-country technician response and stock depth are unconfirmed.
Nipro Surdial X
Alternative where the local package wins
- Best for
- A program supported by a dependable Nipro technical and consumables team.
- Why consider it
- The manufacturer documents HD and online HDF workflows and publishes model-specific training resources.
- The trade-off
- No evidence here establishes lower failure rates or cheaper delivered treatment than the alternatives. Confirm actual licensed/configured modalities.
- Price & package
- Quote required on the same workload and contract scope as competing offers.
- Support
- OEM product/training resources are published; those are not a substitute for local field service.
Budget for the complete service.
Cost per delivered treatment = (annualized machines + water/power installation + service + staffing + testing + consumables + waste + downtime) ÷ treatments actually delivered.
- Treatment volume and utilization
- Dedicated lines, concentrates and filters
- Water-system installation and recurring quality control
- Technician response, spare parts and backup station
- Supply continuity and import logistics

Questions to resolve
- Which provider can document service response and consumable replenishment at the hospital?
- What is included in the per-treatment basket and what changes with the chosen modality?
- Can water, power, clinical staffing and backup arrangements sustain the planned workload?
What the evidence does not establish
- No defensible current public installed-package prices or comparative failure rates established.
- Country availability and support differ.
- No clinical modality or patient treatment recommendation is made.
